Solihull
The Health facilitation team in Solihull support adults with a Learning disability. They can support with health promotion, interventions, and treatment and can provide accessible easy read information to ensure a better understanding of health issues. They work closely with health professionals such as G.P.’s, Dentists and Hospitals to help them understand the needs of people with a Learning Disability. They liaise with G.P.’s to ensure Annual Health checks are being carried out. They can support with hospital admissions/outpatient appointments to communicate between medical staff and families or carers. They ensure reasonable adjustments are put in place and that our service users have access to equitable health services.
