If you live within the practice boundary (see map opposite) and would like to register with the practice, please click on the link below, complete all forms and drop back at the surgery in a sealed envelope into the letterbox at Croft Medical Centre.  Alternatively please pop into reception who will happily supply you with registration forms.

You will be allocated an Accountable GP when you register, they will be responsible for overseeing you care whilst you are registered with us.

New patients have the right to express a preference of doctor on their records. However, the practice may not always be able to comply with such a request. In that event we would explain why. All patients may consult with any doctor working at the practice as every doctor working for the practice is able to access your full medical record.

If you wish to pre-register click on the links below to open the forms. When you have completed all of the details, click on the "Submit" button to mail your form to us. When you visit the surgery for the first time you will be asked to sign the form to confirm that the details are correct.  Any organ donation information on the pre-registration form will not be forwarded unless you have come into the practice and signed the form.

If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.

You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.

To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.
